Baclofen is a centrally acting antispastic agent used to reduce spasticity from conditions like multiple sclerosis and spinal cord injury, and it can be delivered orally or intrathecally when severe or refractory. It is a selective GABA-B receptor agonist that decreases excitatory neurotransmitter release and suppresses mono- and polysynaptic spinal reflexes, easing muscle hypertonia and spasms. Oral baclofen is well absorbed but crosses the blood–brain barrier modestly, while intrathecal delivery targets the spinal cord with lower systemic exposure. Research interest also includes its potential role in alcohol-use disorder via mesolimbic pathway modulation.
