Glycopyrrolate is a synthetic quaternary ammonium anticholinergic agent that competitively inhibits muscarinic receptors, primarily blocking acetylcholine effects at parasympathetic sites in smooth muscle, secretory glands, and the central nervous system without crossing the blood-brain barrier. Clinicians use it to reduce salivary, tracheobronchial, pharyngeal, and gastric secretions, counter reflex bradycardia, and reverse muscarinic effects of cholinergic drugs like neostigmine during anesthesia. It treats primary axillary hyperhidrosis topically in patients aged 9 and older, chronic drooling in pediatric neurologic conditions such as cerebral palsy, and supports peptic ulcer therapy as adjunctive treatment. In COPD, inhalational forms maintain airflow by reducing bronchoconstriction.
